Your first visit to our office establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.

What to Bring

  • Health insurance information including authorizations or referrals
  • List of prescription medications and dosages
  • List of medications that you’re allergic to
  • List of questions you may have
  • Recent test results related to your condition
  • Paper and pencil to take notes

There’s also background information about our committed staff and our first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Medical Records

How do I obtain a copy of my medical record?

The first step to obtaining a copy of your medical record is to fill out a written request that states that we have permission to release your medical records.

After the release is received, the records are processed and forwarded to the designated party. If you would like to obtain a copy of your medical records, please contact our medical records department at (908)-353-1777 to help expedite your request.

Parking Information

Ample parking is available at Trinitas Regional Medical Center. Each of our facilities are designed with the orthopaedic patient in mind including convenient covered drive up access and ground level entry into our office. There are wheelchairs available to assist ambulating patients into all of our office locations, please just ask a staff member for assistance.
